Sponsors: What Forms Do You Need to Fill Out?

Has your apprentice completed all requirements for certification?



If your apprentice has completed all requirements for one certification and is ready to be signed off and move to the next level of certification, all you need is a single form called "Request for Recommendation for Certification for Professional Cook". Contact go2 to obtain a copy of this form.

Registering your apprentice for the first time? Follow these three steps!

If you are registering your apprentice for the first time, complete these three forms in sequence.

Step 1: Register



Apprentice and Sponsor Registration Form

Sponsors and apprentices fill out a separate Apprentice and Sponsor Registration Form for each level of Professional Cook. Why? Professional Cook 1, 2 and 3 are three separate programs.

Step 2: Report Workplace Hours



Work-Based Training Report

As your apprentice completes his or her work-based training, you must report their workplace hours to the Industry Training Authority (ITA) on the Work-Based Training Report. Recording work-based training hours is a requirement that can often be missed. Ideally, you should set a time frame (e.g. every quarter or six months) to turn in your apprentice's hours to the ITA. This way, everything is in place to complete the process.

Step 3: Recommend Your Apprentice for Certification



Recommendation for Certification Form

This form comes directly to you from the ITA once your apprentice has completed all of the certification requirements. It comes to you, the sponsor, because you are the only one who can judge whether your apprentice is ready to be certified. If your apprentice has completed his or her technical training, has the required work-based hours, has completed the practical assessment and final exam—and is ready to move on—ensure this form is signed by yourself as the sponsor and a qualified tradesperson, and send it back to the ITA Customer Service. By completing this form, you are attesting that it is the judgment of a Professional Cook 3 or Red Seal Cook that your apprentice is now working at the skill level of a Professional Cook 1, 2, or 3.

Once the completed Recommendation for Certification form is sent to the ITA, the appropriate certificate will be issued for you to present to your apprentice. Please allow two to three weeks to receive it.
