ITA Glossary of Forms

ACE IT

ACE IT is an industry training program for high school students with courses that give credit for both high school graduation and toward completion of an apprenticeship program. There is no ITA form for a student to fill out as registration is done through the high school career advisor or counsellor or culinary arts department.


Application to Challenge Inter-Provincial Examination

Occupation specific 10 to 11 page set of forms used to challenge either the Inter-Provincial Examination for Cook and Baker, or the Examination for Meatcutter. All documents must be originals and must be submitted in English or translated to English by a Certified Translator.

Fees: $120.00 for assessing documentation of required work experience. No fee for first Certificate of Qualification or IP exam and first re-write. Second re-write, $100 re-write fee. Applications submitted by mail may be paid for by cheque or money order made payable to Industry Training Authority or by Visa, MC or American Express. Debit card is only offered at the Customer Service counter in Vancouver.

Please be sure all boxes are completed or marked N/A, for not applicable, and that all required applicant and employer signatures are on the documents.

Apprentice and Sponsor Registration

One page form that is used to:

  • register a new apprentice
  • update/change information for an existing apprentice

Be sure all boxes are completed or marked N/A, for not applicable, and that both the sponsor’s and apprentice’s signatures are on the document.

To find the correct name of the Industry Training Program (Trade) please review the Trades Program List and select the appropriate program, making sure the correct title is entered on the registration form.

The Registration form can be mailed, faxed or e-mailed to ITA when it is sent in on its own. However, when also sending in the Prior Work-Based Training Credit Application, which needs to be all original documents, the registration form will need to be the original document as well. Please photocopy all original documents and retain for reference.

Once an individual is registered with the ITA as an apprentice they will receive a Registration Number that uniquely identifies them and an Apprenticeship ID that identifies the trade they are registered in. It is possible to have more than one Apprenticeship ID number, i.e. registered as a cook apprentice and then moved over to meatcutter apprenticeship. Please keep your Registration Number easily accessible as all records will be linked to it.


Authorization to Release Personal Information to a Third Party

Three page form that allows the ITA to discuss and/or release specific information about an apprenticeship (see form for more detailed description of information excluded) to someone other than the apprentice. Parents, legal guardians and/or spouses and partners are considered third party individuals and are not allowed access to ITA records without the written consent of the individual.

The Release form can be mailed, faxed or e-mailed to ITA and must be signed by a witness 19 years of age or older and someone other than those named as specified third parties.


Grant and Tax Credit Information and Forms

The federal and BC governments both recently announced tax credits for employers and apprentices participating in eligible apprenticeship programs. The federal incentives apply to Red Seal apprenticeships, including the hospitality-designated Cook and Baker professions. British Columbia has its own non-Red Seal program for Meatcutting, which offers the same incentives as the Federal program.

Apprenticeship Tax Credits: New Federal and Provincial Incentives for Employers will provide employers with detailed information and links to: the federal Apprenticeship Job Creation Tax Credit; the BC Basic Training, Completion Training and Enhanced Training tax credits.

Attention Apprentices: Are You Aware of Government Grants and Tax Credits? will provide apprentices with detailed information and links to: the federal Apprentice Incentive Grant; the BC Basic Training, Completion Training and Enhanced Training tax credits; Tradesperson’s Tools Deduction; and Employment Insurance (EI) benefits for traveling to technical training.

 

ITA Designated Trainer Application Form

Three page form that may be filled out electronically or manually and mailed or e-mailed to ITA. A $350.00 application fee must be submitted with the application and applicants are expected to cover all direct costs associated with the designation review. Prior Technical Training Credit Application

Two page form that is used to apply for credit toward an apprentice’s current trade for previous technical training.

All documents must be originals and at least one of the following types of documents must be submitted for each technical training program to be assessed:

  • Original, Official Transcripts – official transcripts signed by the appropriate authority from the training organization (List of Authorized Technical Training Providers – Cook, Baker, Meatcutter) attended and either sent directly to ITA by the training organization or placed in a sealed envelope to then be sent in by the applicant.
  • Original Letter from Apprenticeship Authority – original letter from an Apprenticeship Authority indicating that the technical training completed has been accepted for Apprenticeship Level Credit by a Provincial/Territorial Apprenticeship Branch, documentation for training completed outside of British Columbia may also be accepted.

The application will not be assessed until all documentation is received, so it is recommended that original transcripts or letter from apprenticeship authority is obtained before application is made.


Secondary School Apprenticeship (SSA) Scholarship Application

This is a four step on-line application process for a $1000 scholarship that the Secondary School Apprentice would complete with the School District SSA Coordinator. The Scholarship Application explains the eligibility requirements.


Work-Based Training Report

One page form completed by the Sponsor to record the number of hours of work-based training under the supervision/direction of a certified tradesperson or equivalent for an apprentice during a specific period of time. This is the form that a sponsor will use to record hours after the apprentice is officially registered with the ITA.

If an apprentice is working for only one employer and they are the Sponsor, the employer will fill out and sign the boxes for Name of Reporting Sponsor/Employer.

If an apprentice is working for more than one employer, the employer that the hours of work-based training are being reported for will complete and sign in the Name of Reporting Sponsor/Employer boxes and the employer that the apprentice is registered with will complete and sign in the Name of Current Sponsor boxes.

A separate Work-Based Training Report will need to be filled out for each employer an apprentice wants to receive work-based training credit for. It’s also recommended to report apprentice’s hours every 3-4 months to keep them accurate and current.

Please be sure that all boxes are completed and that the appropriate Industry Training Program (Trade) name is entered, you can check the Trades Program List to select the appropriate program.
