Incident Investigation Tracking Form & DatabaseBETA The Ski Area Occupational Health & Safety Technical Advisory Committee identified a need to collect standardized root cause data across BC's ski areas to allow for more effective tracking and data analysis. The Incident Investigation Tracking Form is an electronic form that each ski area can use to accurately track injury and near-miss data for their own organization. The objective of this resource is to aid ski areas in identifying key issues within their own workplace and ultimately assist them in developing strategies to create effective, corrective actions to reduce injury rates, durations and claims costs. This form has pop-up help windows, comment boxes and check boxes; each answer cell has a function where the drop-down menu automatically narrows down options based on what you type in the cell (e.g. "Instructor" would pull up Nordic Instructor, Ski Instructor, Bike Instructor, etc.). This form feeds information into an easy-to-use data collection system, namely, an Excel document. No report with queries has been developed at the moment. The end user can sort through the data based on the answers provided in the form (e.g. filter buttons on the top of the columns for ease of analysis). The form also includes a User Guide and a FAQ's page. The form is based on HBC's incident investigation report, with required changes to customize such resource for the ski area sector.
